An independent after-action review has exposed a cascade of security failures at Utah Valley University that preceded the assassination of conservative activist Charlie Kirk on 10 September 2025. Kirk, 31, was shot by a sniper from a rooftop over 400 feet away whilst debating students before approximately 3,000 people in an outdoor courtyard. The university failed to conduct a formal security assessment, produce a written action plan, or adequately consider threats posed by surrounding buildings, despite officials raising initial concerns about the outdoor venue. Kirk's family has filed a wrongful death notice and is pursuing legal action against the university, claiming reckless decisions left him exposed.

The report reveals that Turning Point USA, Kirk's organisation, insisted on using the courtyard location because Kirk had previously appeared there and the team was familiar with it, overriding university concerns. Critical failures included the absence of a documented line-of-sight assessment, no pre-event security briefing with stakeholders, and inadequate coordination between the university, Kirk's private security and external law enforcement. Security focus appears to have been misdirected, with text communications between security personnel revealing concentration on the wrong areas rather than rooftop threats. The suspect, Utah resident Tyler Robinson, has pleaded not guilty to aggravated murder and turned himself in the day after the shooting.
