The father of a Treasury employee who took her own life has accused the department of a "complete failure in their duty of care" after a coroner found it had "materially contributed" to her death by failing to follow its own HR procedures. Chloe Moffat, 26, a PA with an "exemplary" employment record, died on 20 May 2025, a day after being summoned to a meeting where she was accused of sharing confidential information about colleagues and repeatedly denied reassurance about her job security. Her father David Moffat says the case exposes a wider failure of safeguarding within a major Government department.

During the meeting on 19 May, senior manager Liz Farmer refused Ms Moffat's requests for support from a friend and told her she "could not predict the outcome," even though the coroner later ruled that Treasury policy meant dismissal was unlikely and a written warning was the probable maximum sanction. Afterwards, Ms Moffat was locked out of her emails and warned not to discuss the allegations, texting a friend "I am going to be fired" before being found dead by her father the next day at her parents' home in Godalming, Surrey. Coroner Anna Crawford has issued a Prevention of Future Deaths report to the Treasury and the civil service head, while Ms Moffat's mother has called the underlying complaint "vexatious" and "office gossip"; the family says they want "redress" but remain undecided on further action.
