An 18-year-old woman from Perth, Australia, took her own life just hours after being discharged from a psychiatric ward, despite having attempted suicide only 25 minutes before staff allowed her to leave. A review into the death of Hailee Hildebrandt, who was being treated at Fiona Stanley Hospital, found serious failings in her care, including that clinical notes had described her as being at "chronic risk" of self-harm and that she had made explicit statements about her intent to kill herself before discharge. Her mother, Stacey Hildebrandt, has demanded urgent reform of mental health services, questioning how staff failed to recognise the crisis her daughter was in.

The review found Hailee left hospital without her medication or belongings and that no family or friends were contacted, partly because she had removed her mother as her next of kin and no phone number was on file. She later sent her mother distressing text messages saying she intended to kill herself; police traced her phone and found her unresponsive. South Metropolitan Health Service has accepted all 21 recommendations from the report, including new processes for identifying support contacts and flagging discharge risks, while WA's Mental Health Minister has ordered a further independent investigation into Hailee's care.
