Jaiden Shehata, an 11-year-old pupil, was fatally struck by a London-bound train while crossing the Bourneview level crossing near Kenley, south London, on his way to school in January last year. An investigator told an inquest he had probably been watching a video on his recently received iPhone, although this did not necessarily mean he was unaware of the danger. The incident has raised concerns about the safety of the crossing, which was classified as high risk.

Jaiden usually used the crossing and was often accompanied by his older brother, but left 20 minutes earlier than usual on the day of his death. The crossing was closed seven months later following a petition by his family, although Network Rail said it had been compliant with its standards and needed council permission to shut it. Evidence also highlighted possible safety improvements, including better signs and miniature stop lights, while an RAIB report cited the unimposing fencing and muddy, sloping path as contributing factors.
