Workers at an Ohio job site replacing a storm drain reportedly warned supervisors that a trench had become dangerously deep, but were told to enter it regardless, according to a new report. Minutes later the trench collapsed, killing one worker and severely injuring two others, prompting a federal safety investigation and the possibility of criminal charges against the company involved.

The US Occupational Safety and Health Administration (OSHA) has since cited the Ohio firm for trench safety violations. One of the workers killed has been named as Jose Alberto Bara, though further details of the incident, the company involved and the scale of the safety failures were not fully available in the reporting.
