An inquest heard that Lauren Jackson, 17, died after taking her own life at Richard Huish College in Somerset, despite appearing to plan for university and having recently begun driving lessons. Staff found her unresponsive, administered CPR and called emergency services; she died in hospital two days later after suffering a hypoxic brain injury.

Lauren had experienced panic attacks, OCD, intrusive thoughts and self-harm from the age of 14, and had attended around 33 meetings with mental health and children’s services. She received therapy and sertraline, but reported in January that its effects were weakening; the college’s safeguarding lead also said she had spoken regularly about wanting to die and described possible methods.
