A four-month-old girl, Kyra Ali Aslam, died from sepsis in August 2022 following stoma reversal surgery at Sheffield Children's Hospital after medical staff dismissed her mother's concerns about her deteriorating condition. The tragedy highlights a serious failure in patient safety, as Kyra's mother Lariece Greaves raised alarms with doctors the day after surgery when her daughter's behaviour changed, but medical staff attributed this to an anaesthetic reaction rather than investigating further. In reality, Kyra's bowel had begun to leak, a complication that ultimately led to her developing sepsis and dying during emergency repair surgery.

The incident occurred on 11 August 2022 when Kyra underwent stoma reversal surgery—a procedure to reverse an earlier stoma (a surgically created opening) that had been necessary shortly after her birth. By shortly after midnight on 12 August, Kyra began vomiting, and whilst nursing staff suspected sepsis and alerted medical personnel, a coroner's report found that "insufficient weight was placed" on both her mother's concerns and the nursing staff's warnings. The hospital trust subsequently acknowledged failures in identifying and acting upon suspected signs of sepsis, recognising that with appropriate medical care, Kyra would in all likelihood have survived.
