Prison watchdog found no clear warning signs before Peter Lynch’s suicide

Daily Mail ·

Peter Lynch, a 61-year-old father of four, died by suicide in HMP Moorland two months into a sentence for violent disorder during the August 2024 Rotherham riots. A Prisons Ombudsman report said he felt a strong sense of injustice about being jailed and had been tearful, but found little to indicate that he was at heightened risk or that staff could have foreseen his death.

A mental health nurse assessed Lynch on 24 September; he said he felt safe and supported, and denied thoughts of suicide or self-harm. Two days before his death, he was placed on report over alleged threatening and racist language, and received details of a disciplinary hearing the evening before he died. Staff found him in his cell at 6.24am on 19 October 2024, but entered five minutes later while awaiting authorisation; an inquest concluded that he died by suicide.

  • Peter Lynch died by suicide in prison two months into his sentence.
  • He had described feeling a strong sense of injustice about being jailed.
  • The Ombudsman found staff had little warning of heightened suicide risk.

Both sides, in good faith

The strongest fair case each way — we don't pick a winner.

The case for

The report’s finding that there were no clear warning signs matters when judging whether staff could reasonably have anticipated Lynch’s death. He told a nurse he felt safe and supported and denied suicidal thoughts, while the available information showed distress and a sense of injustice but little indication of heightened risk; staff must make decisions on evidence they can reasonably identify at the time.

The case against

A person can be at serious risk without expressing suicidal thoughts, and tearfulness, perceived injustice and the stress of a disciplinary process may warrant closer attention in custody. The five-minute interval before staff entered the cell also raises a legitimate question about whether procedures allowed an adequate response, even if the report found no clear basis to foresee the death.
