An official inquiry has confirmed that whilst no criminal theft or misuse occurred, systematic breakdowns in financial management and monitoring permitted the discrepancy to accumulate undetected throughout the period.

The investigation traced failures to multiple factors including under-resourced finance staff lacking necessary expertise, embedded flawed budgeting assumptions that became routine practice, and inadequate training for oversight committee members. An external treasury advisor had previously recommended a deeper financial review to address visible anomalies, but this recommendation was not pursued, leading investigators to question whether current public-sector audit frameworks are equipped to identify building financial risks before they escalate.
