New details describe “Patient Zero” as a gothic psychological thriller starring Swift, Dakota Johnson and Colin Farrell, with Cara Delevingne appearing as another woman. The video follows Johnson and Farrell through a deteriorating relationship, while Swift appears as a warning presence; a scene in which Johnson blacks out after one drink and is then gaslit by Farrell points to the story’s central danger.
Swift premiered the video at the 2026 MTV Video Music Awards on 27 September, where she received the inaugural Artist Director Honors and the video of the year award for “The Fate of Ophelia”. She said the video draws on ghost stories, true crime and psychological thrillers, with its house representing a relationship grown cold. “Patient Zero” is one of four new songs released with *The Life of a Showgirl: The Encore*, and the video is available on Spotify, Apple Music and Amazon Music, but not YouTube.
