A safety investigation has highlighted serious deficiencies at a Sheffield tram crossing following an incident in June 2025 where two pedestrians were struck by a tram travelling at 17mph. One person sustained serious injuries whilst the other received minor injuries when crossing at Staniforth Road in Darnall. Investigators determined that pedestrians failed to notice the approaching tram because audible warnings were insufficient and the crossing layout and signage provided inadequate hazard communication.

The Rail Accident Investigation Branch found that the tram operator and combined authority failed to manage safety risks effectively at this location. The watchdog has issued three recommendations: conducting a comprehensive safety review at the specific crossing, ensuring consistent use of warning procedures by all tram drivers across the network, and assessing pedestrian safety risks at every location where tram strikes are possible. The investigation also noted that a raised cobbled surface immediately beyond the crossing may have worsened the severity of injuries sustained.
