A 24-year-old demi chef from the Philippines died after jumping from the 100-metre-high Sky Deck at The Star Casino's Queen's Wharf development in Brisbane in June, having accessed the platform via a handrail beside a gap in the glass safety barrier. Emails that emerged after her death, first reported by The Sydney Morning Herald, show that Star and its Queen's Wharf partners had received repeated warnings about this exact safety flaw from senior staff over the preceding months but failed to act.

The first written warning came from a senior Star manager in May 2025, who flagged the handrail balustrade as an obvious risk that could let someone climb through the safety glass gap on the 23rd floor. Further warnings followed in July and September 2025, and an independent report dated 14 October cautioned that "recent and unfortunate incidents" showed the balustrade handrail could be misused as a hand and foot hold, urging urgent modifications such as removing the handrails. Only after the employee's death did The Star hire security and install temporary hoarding, though no permanent fix has yet been completed; the company has been contacted for comment.
