The incident occurred in May, when Grayson entered the coffee shop to complain about a mistake in her order and became embroiled in a brawl with staff; she was found unresponsive on the floor and later pronounced dead at a nearby hospital. The findings matter because they clarify the medical cause of a death that had drawn public attention and prompted allegations from the woman's family.

The Allen County Coroner's Office ruled that Grayson died from an "acute exacerbation of congestive heart failure in the setting of physical altercation," and classified her manner of death as "undetermined" — meaning it could not be established whether the death was natural, accidental or a homicide. The autopsy noted "no significant contributory injuries," indicating that any physical trauma sustained in the fight was minor and did not cause her death.
