How coordinated care can help seniors and vulnerable individuals find independence and purpose
Two care recipients who moved from a welfare home into independent community living illustrate how personalised support can help people build confidence and take greater control of their lives. The story highlights 4S’s approach, which combines individual care plans with relationships, community involvement and opportunities for people to contribute.
Ms Rose and Ms Lilac prepared for their move by sharing a room, learning to budget and manage household bills, and practising how to get around Singapore. Rose now manages her medication and works as a cleaner, while Lilac works in a café; they moved into a rental flat in April. In 2025 and 2026, 4S supported 665 welfare home residents and 993 clients at its ageing and eldercare centres, alongside partnerships that provide digital skills and other support.
In short
- Tailored support helped two former residents move into independent living.
- Rose and Lilac prepared through budgeting and practical life skills.
- 4S supported 1,658 people across its homes and centres in 2025 and 2026.
